DOCUMENTATION

  • CLEAR: All documentation needs to be written as if the person reading it has never worked with the client but is able to smoothly transition into the clients team and support the client’s goals and needs.
  • CONSISTENT: All documentation should contain the same information.  Contradictory information creates confusion and hinders successful support.
  • ACCURATE: All documentation must have accurate information regarding the client, family, and team as well as being grammatically correct. 
  • UP TO DATE: All documentation must be kept up to date to show accurate growth, success, and challenges.  

EVALUATION

I learn about the client's wants/goals and needs/challenges through working with the client, family and support team.  This process highlights current challenges and barriers hindering the client’s success in any particular setting.  

What is the client’s current living situation, work environment, and community involvement? What methods of support have been tried and how successful have those methods been?

Through observation in a variety of settings and evaluation of current methods, I am able to make suggestions for changes in activities and supports to increase client success..

WHAT I DO

  • I review all current documentation from all sources including school documentation (IEP, BIP/BSP, SOP, Evaluations, etc). I make recommendations for updates and changes based on my findings.
  • I perform a current functional behavior analysis (FBA) and document my findings.
  • I use the FBA findings to create an individualized and effective support plan (BSP/BIP).
  • I create a positive personality profile (PPP) to make sure all team members see the client as a whole individual with strengths and challenges just like all individuals.
  • I have written “All About Me” communication books to share with new support people before they provide direct services to increase success and happiness. 
  • Families and clients often need documentation for new agencies or for legal purposes.  I write the letters and provide the necessary documentation showing support needs and levels of support required.
  • I attend IEP or other meetings to help the client and the family ensure that needed support is provided and that documentation is accurate, clear, and up to date.
